Camper Registration

NOTE:

Please send in your registration as soon as possible. Our deadline for camper registration is July 1, 2024. No refunds will be given after this date. If you are paying by credit card, we are unable to refund your processing fee.
All forms are due ASAP with the exception of the BSA Health and Medical Record in the case where, due to insurance regulations, the camper or leader cannot have their physical completed by July 1. In that case, please submit as soon as complete.

(NOTE: When filling out the age and grade of your camper, please enter the age/grade they will be entering by August 1, 2024)

MEDICAL FORMS^

BSA Health & Medical Record (Required for all campers and leaders. Part C must be signed by physician).
Medical Form Checklist (Use this when completing the BSA Health & Medical Record.)
Medical Exemption Form (Complete if requesting an exemption from BSA vaccine & immunization requirement.)

ATV PARTICIPATION AGREEMENT (AGES 14+)^

BUG NET RENTAL FORM^

We have a very limited number of bug nets available to rent for the week. It is recommended you complete and return the rental form with payment as soon as possible.

Camper Packing List (suggestions)

* BSA Post Membership Forms are required for your application to be complete. These forms provide insurance for your camper while they are at Camp COLEY and provide background checks for our adult volunteers. Please let us know if you have any questions. Certain fields of the form will be filled out by the office and are thus expected to be left blank. Those fields are:

Signature of post advisor
Participation fee
Exploring Post/Club and Number
