To register for Camp COLEY, please complete the registration form below
and send with a check payable to Camp COLEY to: Camp COLEY c/o Gladys Purvis 9 Wyndmoor Drive Glenside, PA 19038 Please note: - The Registration Form, Driving Permission Form (if applicable), and Medical Form must all be mailed in advance. Parts A, B and C of the medical form need to be completed and signed by a doctor.
- When checking in, please have your camper’s completed Information Form and Three Strikes Form. If your camper will be taking OTC medications at camp, please also bring the completed medication form. The medication form must have a doctor’s signature.
- Parents are to hand in all medications, in the original container, directly to the health officer before leaving camp. Also, please be sure to pick up medication at the close of camp. New Jersey State law mandates that all mediation left at camp must be destroyed within three days and cannot be mailed.
